To be eligible to participate, you must complete a survey and a census and submit them. Nonmembers can complete the survey and census. Submitting this information does NOT obligate you to participate in the health plan nor join NCLifeSci. Jan. 1, 2027, is the expected start date of the plan.

• You DO need at least two employees.
